Registration for the 2022-2023 School Year - School Starts August 23
How to register returning students: 
A - Online registration through JMC is the recommended method.

JMC Online registration needs to be done on a computer or tablet, it is not compatible with phones or small screen tablets.
However not everyone has internet access, to schedule an appointment to use a school computer in the board room, please call 563-2607.

This takes place of the Student Information Form, Technology Permission Form, and the PK-4 After School Plans Form.  
Fees can be paid through JMC
 or money may be sent to the Business Office.

School Registration Fees:      K-5:  $25     ~    6-12:  $50     ~     PK-2 Snack Fee:  $70
Breakfast: $2.10, Lunch $2.90    (PK-4 students will have free meals through the Community Eligibility Provision)
Snack or Extra Milk:  $.50
Student Activity Ticket:  $50 (or $5 gate admission; $4 for Football);           
Adult Activity Punch Card: 10 punches for $45 (or $5 gate admission; $6 for Football)

B - Mail/Email registration 
Print the forms below, fill out & mail or email them to the school
Mail to: Audubon Comm School - 800 Third Ave, Audubon; email them to:
Fees may be paid through JMC or send to the school, below is a fees receipt.
C - Walk in registration is August 9 (4pm-6pm)
Pick up blank forms or drop off filled out forms and pay school fees
 (You will not be able to sit and fill out paperwork or use laptops for online during this time.)

Health Screening Requirements
All kindergarten & freshmen  Form to be signed by your dentist.

Certificate of Vision Screening (pdf)  - 3rd Grade & Any Kindergarten that did not get the KidSight screening done

Immunizations up to date--check with your family doctor and bring a copy after any new shots for school records.

ATHLETICS:  All 7-12 students who plan to participate in any sport, cheerleading, and/or dance team will need a signed physical form and a signed Heads Up Concussion form.

Public Release  Housing Assistance

If you are direct certified through the state database or receive the SNAP Lunch Letter, then you
don’t need to fill out the entire application, however if you would like to receive assistance
for book fees and/or drivers education fees you need to complete this waiver.
