The jr high jersey order form is due at NOON, August 11th - all other forms are due on or before the first day of school, Aug. 23rd.
The on-line registration is available on JMC Parent Access under Student Registration, which would take place of the Student Information Form, Technology Permission Form, Heads Up Concussion Form and the PK-4 After School Plans Form. Fees can be paid through PaySchools (starting July 1)

ATHLETICS: All 7-12 students who plan to participate in any sport, cheerleading, and/or dance team will need a physical form. The 'Heads Up' concussion form is now included in the registration form. If you would like to read the fact sheet on concussions, click the form below. It does not need to be signed & returned if you've indicated yes on the registration form.
7-8 participants will also need a Jr High Jersey Order Form if they don't already have a school reversible jersey.
